hidden submakes in doc

Description

If doc/Makefile is called with -jN (N >=2), one get warnings from make, as doc2tex invokes make unexpectedly.

Attached patch marks all DOC2TEX invocations as possibly sub-make spawning, by prefixing a '+'.
